This program provides housing assistance to low-income individuals (and their families) who

have a medical diagnosis of HIV/AIDS. • Tenant Based Rental Assistance: Helps low-income families with their

rental payments. • Short Term Rent, Mortgage and Utility assistance: Helps individuals and families who are at

risk of becoming homeless stay housed by providing rental, mortgage or utility payments and increasing access

to supportive services. • Permanent Housing Placement: Helps unhoused families move into permanent housing

by assisting with: » Application and credit check fees » One-time utility connection fees » Rental security

deposits » First and last months’ rent.

Linkages is a tenant-based, permanent supportive housing program for homeless adults who are diagnosed with

severe mental illness (SMI) and are also functionally impaired. As the subcontractor overseeing Housing

Administrators, the New Mexico Mortgage Finance Authority (MFA) provides rental assistance for extremely

low-income households in 6 sites throughout NM since 2007. As of FY21, Linkage’s programming is projected to

serve over 300 households annually with a Linkages voucher (rental assistance) but depends upon household

size and turnover.

These support services subcontractors, 11 providers throughout NM, are overseen by BHSD staff. The providers

screen for eligibility and refer Linkage’s applicants to the Housing Administrator; assist consumers to obtain and

maintain their supportive housing by providing required supportive housing services including engagement,

service planning, crisis prevention and intervention, resource coordination, skill building, symptom management

 food boxes and clothing are provided on Thursday at 117 E Youth St Farmington NM, 87401.

Services Provided: Economic Council Helping Others, Inc. provides numerous services to low-income

individuals and families in San Juan County. These services include the following:

Commodity Supplemental Food Program (CSFP) - assists income qualified seniors 60 years or older, monthly,

with nutritious foods, basic nutrition information and recipes. Also, Senior Care Kits for Incontinence are

available.

The Emergency Food Assistance Program (TEFAP) - is provided monthly in emergency food boxes for all San

Juan County residents with low-income. Some types of Food Boxes are Food and Nutrition (Fan) monthly special

diet boxes. These are provided to individuals on specially prescribed diets (such as medically fragile, diabetes,

etc.), for persons who cannot qualify for other programs. (Please provide a letter from your family doctor, proof

of physical address, proof of household income and a Picture I.D). Holiday Food boxes – Merry

Christmas/Thanksgiving food boxes are available during holidays. (Please bring your Picture I.D for adults and

proof of physical address). Also bring any other legal document for minors, proof of physical address &

household incomes.

Admission Process: A free, confidential mental health assessment provided on by appointment or on a walk-in basis.

Assessments are available at the center, 24/7. No referral required.

Services provided: Inpatient Programs are structured to meet the mental health and chemical dependency concerns

that are troubling adolescents. Our acute care programs provide stabilization for those suffering from

disruptions in their daily lives due to emotional, behavioral and/or substance use disorders. Patients receive

individual treatment in areas such as: medication management and education, case management, recreational

and art therapy, relapse prevention and discharge planning.

Available to: Anyone with desire to end narcotic addiction.

Group therapy and 12-step programs such as NA meetings have been helping people get, and stay, sober for decades. These groups are held every day, everywhere. Find support groups in your own community now, and get the help you need to maintain a happy, healthy, and sober lifestyle.

The facility provides a time-limited medical detoxification program designed to assist the persons

suffering with the physiological effects of acute withdrawal from alcohol and other drugs. Inpatient, Residential or

Ambulatory Care. Staff motivates the individual to continue with an inpatient treatment program, after their

discharge. Lesson to help them to progress toward a full and complete recovery. The program is adequately staffed

to ensure biomedical and psychosocial assessment, observation and care, and referrals to meet the individual’s

needs. We offer a distinct program that provides culturally and linguistically appropriate services. The program

operates 24/7 for the purposes of providing a safe, monitored, and managed detox treatment. The typical stay is 3-713
