We provide Sexual Assault Nurse Exams (SANE – medical/forensic exams), rape crisis advocacy, and counseling. Our staff also works in the community to prevent sexual assault through multi-session, education-based programs with specific groups. This education includes awareness programs, including ‘Safe Dates’, a healthy relationship curriculum, geared for high school students.
SAS will be will you throughout every step of your recovery process, from an exam to counseling, and facilitation of legal services to pursue the person who assaulted you. We lovingly operate support groups and educational group interventions to encourage conversation about difficult topics. We offer a food assistance program, on-site medical clinic, transportation assistance for medical appointments, health insurance navigation and more.
We run stigma-free syringe exchanges, free HIV, syphilis and hepatitis C testing without any judgement, and a PrEP linkage program that guides people in need to the prevention methods that work for them. Our MAT (medication assisted treatment) linkage helps healthcare providers be able to use evidence-based treatments for people who use substances while also providing people who use substances information they can use to find treatment.
We operate housing assistance for qualifying individuals throughout New Mexico to see people moving from the streets into homes that foster healthier living opportunities. Funding from federal and state sources allows applications to receive rental assistance, utility assistance, and more.

IMMUNIZATION CLINIC: held in Bloomfield, call for appointment.
FAMILY PLANNING SERVICES: held in Bloomfield and based on a sliding scale fee. Offering annual exams, pap smears, birth control methods, counseling, pre-pregnancy planning, pregnancy tests, and emergency contraception. Call for appointment.
SEXUALLY TRANSMITTED DISEASE SCREENING AND TREATMENT: call for appointment.
HIV TESTING: Confidential or anonymous screening, treatment, and counseling. Call for appointment. TUBERCULOSIS, SCREENING AND FOLLOW-UP: We screen contacts of active cases of tuberculosis, and foreign-born individuals. Call for appointment.
FLU SHOTS: Target population is individuals over age 65 and/or people with chronic illness. Held in late fall in Bloomfield, Aztec and Blanco.
COMMUNICABLE DISEASE FOLLOW-UP: We follow up on cases of
reportable diseases such as salmonella, hepatitis, whooping cough, measles, and tuberculosis. Services are available to individuals with reportable diseases.
WOMEN, INFANTS, AND CHILDREN (WIC) NUTRITIONAL PROGRAM: Services are available by appointment and eligibility is based on family income and size. We serve women with children up to age 5, pregnant women, and postpartum women. Families FIRST Program: Families FIRST is a free program offered by the New Mexico Department of Health. The program provides case management services by registered nurses for Medicaid-eligible pregnant women and children up to 3 years of age. Families FIRST improves pregnancy outcomes and promotes healthy children and families by connecting clients with medical, social, and educational resources in the community. Families FIRST helps with obtaining: Prenatal care, Medicaid, SNAP, TANF and WIC benefits, childbirth and parenting classes, car seats and transportation to medical appointments and referral to community resources. -
Available to: Birth to 21 and adults with cystic fibrosis or metabolic disorders
Services Provided: CMS offers care coordination, specialty clinics and medical coverage for persons ages birth to 21, with chronic illnesses or medical conditions that require surgical or medical treatment and limit their activity. Eligibly conditions include diabetes, cardiac, cancer, cleft lip and palate, among many others. Specialty clinics are available by referral for asthma, cleft lip and palate, genetics, metabolic disorders, neurology, endocrinology nephrology, and gastroenterology. There is a financial eligibilityrequirement for medical coverage, except for diagnostic services and Adult Cystic Fibrosis. Medical coverage is further limited to persons who are not Medicaid-eligible.

help families break the cycle of dependency on public assistance. The Human Services Department also recognizes the importance of developing work readiness in the populations served through public assistance. To do this, HSD has developed programs geared towards the training and placement of individuals into career positions to help make the future a little brighter. The Human Service Department (HSD) serves over 800,000 New Mexicans each month by administering several large state and federally funded programs with

Connelly Hospitality House: We provide lodging for cancer patients and their caregivers who travel 50 miles or more to receive cancer treatment in Farmington, NM.
Families of those hospitalized with a traumatic injury or medical emergency and who meet the distance requirement are also welcomed when rooms are available.
We also have 3 RV spaces with electricity, water, and sewage access.
Cathy Lincoln Memorial Cancer Fund: This fund is designed to provide financial assistance to women, 18 years and older, of San Juan County, NM.
Its purpose is to assist women financially with cancer-related health services. Covered services may include medical exams, medications, diagnostic testing, and treatments.
Recipients can reapply each year.
Riley Men’s Health Fund: This fund is designed to provide financial assistance to uninsured, under-insured, and low- incomes men, 18 years and older, of San Juan County, NM.
Its aim is to assist men financially with health related services such as medical diagnostic testing and treatments. Recipients can reapply each year.

We perform noninvasive and minimally invasive tests and procedures that check, support, and improve your heart health. We’ll work with you to develop a personalized treatment plan and treat you with the most effective medications. Our services include:- Evaluating you for coronary heart disease, heart-valve disease, and heart defects
- Determining how well blood flows through your heart vessels or find the cause of your heart failure
- Treating your heart vessels with nonsurgical or minimally invasive operations that open and support them through balloon angioplasty or stent placement

All Veterans who have a primary care provider have a social worker assigned to them. This is their Patient Aligned Care Team (PACT) Social Worker. Veterans can call and request to speak to their PACT Social Worker or they can walk-in where they check-in to have their primary care visit and request to see their social worker. No referral is required. If you are part of a specialized program such as HUD-VASH or Military to VA, the PACT Social Worker will help you contact your assigned case manager.
Social work is integral to health care and in helping Veterans have additional support that can improve their quality of life. We can help you and your family manage stress related problems due to illness, injury, and challenges that life throws your way. Areas of assistance include but are not limited to:

Healthcare Assistance Program (HCAP)
Purpose as set forth in NMSA 1978 §27-5-2:
- Otero County is the responsible agency for ambulance transportation, hospital care, or the provision of health care to indigent patients domiciled in Otero County.
- Otero County provides support to the State Medicaid Program.
- Otero County can improve the provision of health care to indigent patients with local revenue for countywide or multi-county health planning.
Disposition Program
Purpose as set forth in NMSA 1978 §24-13-1:
- Otero County is the responsible agency to ensure the body of any unclaimed decedent or indigent person is decently interred or cremated

Presbyterian Medical Services participates in the 340B Drug Pricing Program, which allows us to obtain discounted prices on covered outpatient drugs from drug manufacturers. The 340B program allows our health centers to ensure patients can afford prescribed medications.
Benefits include access to new, high-cost medications. Many would not be affordable to patients. Even insured patients who have high deductibles and copays can take advantage of the savings offered through the 340B program.
Insurance & Payments We Accept
Everyone is eligible to receive Presbyterian Medical Services community health center services. We accept Medicaid, Medicare, and private insurance. For the uninsured, a sliding fee scale is available based on income. Our health care guides provide on-site Medicaid or Health Insurance Marketplace enrollment assistance.

Pregnancy Testing
Getting the result from a pregnancy test is a pivotal moment in time. The Pregnancy Help Center will help you as you navigate through your next steps. We provide education, resources, and information on assistance available to you in a caring, confidential manner.
A pregnancy verification will be provided to help you get needed assistance (picture ID is required for this document).
Ultrasound
If your pregnancy test at the center is positive, you may be eligible for a complimentary ultrasound. The nurse will look for the baby’s heartbeat and take measurements confirming if there is a viable pregnancy.
Our ultrasounds are limited in scope and provided in the first trimester of pregnancy.
Building Blocks
Building blocks is an “earn as you learn” exchange. Moms and Dads receive practical education and earn “Block Bucks” to attain baby items in our shop. Items like wipes, diapers, clothing up to 18 months, strollers etc., are available in in our store.

Food Pantry -Open 4 days a week, by appointment, to help provide food for those families in need
Furniture Ministry- Provides household and furniture items to those families and individuals who are in need
Building Supplies-Partner with Home Depot to provide building material to repair homes in Otero County
Medical Equipment Ministry-Loans medical equipment to those who do not have insurance.
Backpack Food Ministry- Provides weekend backpacks filled with non-perishables to our public school children that are identified as being in need. The backpacks go home on Friday and are returned on Monday to be replenished for the following week.
Benevolence Fund- Provides assistance with things such as prescriptions, utility bills, rent and overnight shelter. In addition, volunteers can assist with small household repair, wheelchair ramps, preparing coolers/heaters, etc.
PNM Good Neighbor-Love INC is the administrator for PNM’s Good Neighbor Fund for Otero County, assisting those that may need help paying their electric bill.
Community Garden-Created as a way of providing healthy, nutritious produce that families that they can grow themselves.
Community Classes-Walk alongside clients, giving them the tools and encouragement, they need to reach their full potential.
Household Ministry-Provides household necessities like dishes, pots and pans, bedding, towels and more.
