• This program serves:
  - Pregnant, postpartum and breastfeeding women.
  - Infants, and children up to age 5.
• This program helps people with income at or below 185% of federal poverty guidelines.

Documents Required

• Identification:Proof of identification
• Proof of Income:Proof of income including Proof of eligibility for Medicaid, SNAP (Food Stamps), TANF (Cash Assistance), and/or FDPIR (if someone in your family is eligible). If your family received WIC services in the past, the WIC EBT card should be brought to each appointment, including application for new services.
• Proof of Residency:Proof of home address in the state of New Mexico (no P.O. box addresses)
